Might have sounded like it was from the instructions, but it's just standard regular checks/maintenance.

It's a moving part, held on by a screw, that also moves. It can come apart.

As others have said, it's not the engines fault, and quite common to happen if overlooked.

Most operators of any type of quipment will regularly check them on some sort of basis.

We have to at work, on all our machines. Especially the rough cutters. (out front mowers)

Not having a go at you mate, it's something easily overlooked.
